Book a new patient appointment
When booking your appointment, choose New Patient - Standard Appointment. As a new patient to our practice, it is important for your GP to have an understanding of your past medical history. Choosing this appointment type will allow your GP the appropriate amount of time longer for your first consultation.
If you have a complex or significant medical history, your doctor may wish to access a copy of your medical record from your previous medical practice.
Medical files can be transferred between practices when patients move. Your previous practice will be able to tell you if there is a transfer fee.
Preparing for your first appointment at our practice
Complete a New Patient Form
Save time by printing and completing your new patient form before you arrive for your appointment. Hand your completed form to Reception when you arrive.
The new patient form covers essential information:
- Your current medications, doses and frequency
- Remember to include any complementary medications such as vitamins and supplements
- Your contact information
- Contact information for your next of kin
- Allergies
- Health conditions
- Immunisation history
- Family history of chronic disease
- Approximate dates of your previous tests e.g. cervical screening/pap smear, skin checks, prostate checks
What to bring
- Medicare card
- If applicable - your DVA card, Pension card or Health Care card
- You may bring a support person or carer with you

- Before your visit, write down any questions you want to ask your doctor
Haven't completed your New Patient Form before your visit?
If you haven't had time to complete the form, that's okay. Instead, arrive 15 minutes early to your first appointment. Our Reception team will give you the form to fill out. when you arrive. Once you hand back the form to Reception, we'll create your new patient folder in our software, ready for your appointment.
How to book
Consultations with the Doctors at our practices are by appointment. We do our best to accommodate walk-in patients.
Let us know if you would like help choosing the right doctor for your needs. All evidence suggests that communication between a doctor and patient is improved when a good relationship exists between the two. We recognise it is not always possible to see the same doctor, so our system of recording notes takes this into account. You can read more about the providers in our rooms on the Provider pages. You can also visit doctors at either one of our practices as we have a shared patient database.
A standard consultation is 10 minutes. A standard appointment can be booked online. Patients with appointments will have preference over walk-ins, except in emergencies.
An appointment for a standard consultation should be made with your preferred doctor for your continuity of care or the next available doctor. Please ensure you advise our receptionist if you prefer a particular doctor or require urgent attention for matters such as chest pain, burns or an eye injury, for example.
If your consultation is for a cervical screening, skin excision, medical investigation, or you have multiple issues, you will need longer than a standard appointment.
Longer consultations can only be booked by phoning our Reception team. This ensures the appropriate appointment length can be allocated to your doctor. If you have booked incorrectly, your doctor may ask you to book an additional appointment to attend the practice another time.
Let us know if your concern is urgent. Urgent medical matters will always be dealt with promptly and we will do our best to work with the providers in our rooms to accommodate your needs.
In a medical emergency, call 000 and ask for an ambulance.
If your appointment is no longer required, please tell us as soon as possible so your provider can allocate the appointment to another patient. You are able to cancel your appointment via our online booking system or phone the practice.
Online Bookings are made via AutoMed by clicking the booking links on our website or using the AutoMed app.
Online bookings are suitable for:
- Requesting repeat prescriptions
- Blood pressure checks
- Follow-up appointments
- Organising referrals
- Organising pathology or radiology tests
- Coughs & colds
- Cervical Screening Test
- Other minor illnesses
- Sickness certificates
Please phone Reception on 07 5578 9000 to make an appointment for:
- Complex or multiple issues
- Skin excisions or surgical procedures
- Centrelink/Insurance documents
- Workcover or other Medicals
- Postnatal checks
- Counselling
- More than one family member to be seen
- Appointments that also need the nurse, e.g. dressings, vaccinations, child health checks, ECG’s, spirometry, ear syringing.
Your visit
Let our Reception team know when you have arrived.
You can also "check in" using the electronic kiosk screen. If you use the kiosk screen, you can confirm your contact details are up to date and view your place in the queue.
If you're late or your appointment is for one of the following reasons, please check in at the Reception desk (not the kiosk screen):
- New Patients
- Workcover
- Procedures
- Vaccinations / Injections
- Care plan paperwork
- Medicals
We do our best to provide appointments on time, however delays are sometimes unavoidable, particularly when patients have unexpected or serious concerns.
There are 3 ways you can help us reduce waiting times:
- Please cancel your appointment if you can't make it.
- If you have several problems, or a complex problem, please phone the practice to book a long appointment.
- If multiple family members need to be seen by the doctor, please book an appointment for each person.
Fees for your appointment
We are a mixed billing practice
Our practice and the providers in our rooms offer private and direct Medicare billing (also known as bulk-billing). The following patients are bulk-billed:
- Health Care Card holders
- Holders of a valid government concession card
- Government Seniors card
- Children aged under 16
A $10 discount is offered to students and those between 16 -18 yrs old.
Gold and white card DVA holders are directly billed to DVA. Private patients may be bulk-billed at the discretion of their doctor.
Fees (from 1 November 2024)
Item 23 – Standard Consultation: $90 (Immediate Medicare Rebate $42.85)
Item 36 – Long Consultation: $130 (Immediate Medicare Rebate $82.90)
Appointments between 7.30am and 8am are considered after-hours and may carry a fee of $95 for private patients. (The rebate is slightly higher)
Private patients may be bulk-billed at the discretion of their doctor.
At times there may be an out of pocket fee related to services, e.g. IUD insertion, Iron Infusions, Procedures etc. Your doctor or reception will let you know at the time of booking.
Medicare Rebates
We offer immediate Medicare claiming on the same day as your appointment for valid Medicare card holders. Please make sure your bank details are registered with Medicare.
Medicare does not provide a rebate for some services and these require payment by the patient on the day. Fees will be quoted by Reception or by the providing doctor. These services include:
- Insurance/Medical paperwork
- Work Cover
- Gap payments for procedures
- Medicals
- Scripts/referrals when the patient does not attend
- Any visit where the patient is not present
Where possible please give reception a brief explanation of your visit reason so they can book you appropriately with the correct provider and nursing team if applicable, and share fees with you before your appointment. This makes it easier at the end of your visit to know what payment, if any, will be required that day.
Specialist Referrals
A consultation with a doctor must happen before a referral to a specialist. Doctors are unable to backdate referrals, they must be dated when the letter was written with the patient present. Before attending a specialist appointment, please check with their reception team to see if new referral is required for the visit. Please allow adequate time to get the referral prior to the specialist appointment.
Repeat Prescriptions
For prescriptions, your doctor will typically provide enough medication to last until you are due for your next review. The providers in our rooms do not issue prescriptions without seeing the patient unless prior arrangement has been made. A fee may apply if a script is requested without consultation.
recallS + REMINDERS
You may receive health reminders by SMS which require you to provide your surname and date of birth to identify yourself before the message will open. You will be able to click a link and book an appointment online from there if you wish. Let us know if you would like to opt out of SMS reminders and we can phone you or send a letter instead.
Medical Certificates
Medical certificates are legal documents and are only issued if you present for consultation. As is the case for referrals, the providers are unable to backdate them.
Results
It is our practice policy that an appointment is needed to obtain your results unless your doctor has indicated otherwise. We do not give out results over the phone as we cannot identify you and the doctor is likely to need allocated time to start further investigation, provide prescriptions and answer any questions you may have.
Depending on your result your doctor may use our secure texting system to let you know your results are okay OR they may ask a nurse to ring you, or ring you themselves. They should make it clear to you when you obtain your referral/request when you should re-book.
Our reception team can only share limited information and this is why we discourage patients phoning the practice to ask if their results are back. Due to privacy laws we are unable to give information to other persons without your permission. Please speak to the receptionist if you would like to discuss this further.
